Medicare Documentation Checklist
What's typically needed to bill Medicare or insurance — we help obtain anything you don't have.
Bring or mention what you have; we'll coordinate the rest with your provider.
Usually required
- Treating provider's written order / prescription
- A recent in-person (face-to-face) visit note — within a set window for some items
- Chart notes describing the diagnosis and the functional limitation (not diagnosis alone)
- Documentation that simpler options aren't enough (e.g., why a cane or walker won't do)
By item — common extras
- Power wheelchair / scooter: in-person mobility exam within 45 days; can't safely use a cane, walker, or manual chair
- Lift chair: form CMS-846; severe hip/knee arthritis or neuromuscular disease; unable to stand from a regular chair
- Wound dressings: wound type, size (L × W × D), and a dressing order
- Therapeutic (diabetic) shoes: diabetes plus a qualifying foot condition and physician certification
- Lymphedema compression: a lymphedema diagnosis and an order
